Excerpt from Genesis Game Guide by The Scribe

NAME: Ka-Ge-Ki - Fists of Steel
AUTHOR/VENDOR: Sage's Creation/Kaneko
RELEASED: 1991
TYPE: Fighting
LANGUAGE: English

PREMISE: An unusual pseudo-3D fighter that eschews the traditional
SF2-style platform approach in favor of superdeformed
characters matched together in ring-style bouts.

IMPRESSIONS: Looks rather comical, but is well done. Takes some
getting used to, though, so you might want to save it
for those days when you're tired of standard fighting
                  fare.

VARIATIONS: Surely a Japanese original exists for this game, although
I have yet to see it. I base this on the use of columnar
texts in the title screen, which run top-to-bottom from
right to left, just like traditional Japanese text would.

I own the Japanese version of this game and would recommend it because it is a change from the usual beat-em-up fare.

I remember playing this game on an arcade machine, at a restaurant (think it was Fat Burger...) when I was visiting California. Anyways, the original arcade version was WAY better than the Genesis version (which looks and sounds worse :). But, back then (and maybe even now, to some ghetto folks) it may have been convenient to have the Genesis version for home use, but now that we have emulators like MAME, you can simply download it and play the original. :P

In other notes, the name is sort of a pun. "Kigeki" means "Comedy" in Japanese, but "Kageki" is a kind of theatrical show, or entertainment. But of course, "Ka-Ge-Ki!" is basically a comical fighting game. Anyways, just play the game. :D
